We are expanding our rehab team and looking for motivated Physical Therapists who want to grow their careers in a supportive, high-performance environment.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Plan, prepare, or carry out individually designed programs of physical treatment to maintain, improve, or restore physical functioning, alleviate pain, or prevent physical dysfunction in patients.
  • Ability to exhibit high levels of emotional intelligence that allows success when working with a variety of patient and staff personalities.
  • Accurately and efficiently examines the patient’s condition and evaluate findings to implement a physical/occupational plan of care.
  • Determine appropriate goals for the patient that is addressed through the plan of care.
  • Provide both written and verbal communication in a timely manner to all departments within the rehab department to insure efficient work flow and accomplishment of work tasks.
  • Professional communication with patients regarding scheduling appointments, plan of care, other needs and requirements.
  • Administer PT treatment through manual and non-manual therapies such as exercise, ambulatory functions, ADL training, assistive devices.
  • Ongoing evaluation of patient’s therapy session and ability to provide effective recommendations, long term plan and treatment.
  • Communication with referral sources in a confident, concise, and respectful manner to best coordinate care and develop collaborative relationships.
  • Complete discharge planning with patients and when necessary work with referring provider and other health care workers.
  • Provide documentation of service to meet or exceed the standards of the companies’ documentation compliance protocol.
  • Completion of documentation in a timely manner (24-48 hours) from the time of service.
  • Insures maintenance of HIPPA compliance standards as well as confidentiality of clinic operations.
  • Maintain OSHA guideline standards as trained from company standards.

Your Role

QUALIFICATIONS:

Knowledge:

  • Knowledge of principles and processes for providing customer and personal services.

Education and Experience:

Fifth year college or university program certificate; or two to four years related experience and/or training; or equivalent combination of education and experience. Current Oregon Occupational Therapy license, CHT.
